QUALIFICATIONS

BA/BS Degree (minimum)

Valid/current New Mexico Teaching License
Bilingual preferred


 Knowledge of research findings related to dropout prevention
 Knowledge and proven experience in Instructional and Behavioral Interventions
 Knowledge and proven experience in working with Students with Disabilities
 Knowledge and proven expertise in working with students and families
 Ability to communicate knowledge effectively to others
 Ability to develop and maintain effective and collaborative relationships with students, families and staff
 Ability to analyze information and data to make informed decisions regarding student interventions
 Ability to create and maintain professional documentation of work
 Proven experience to work collaboratively to accomplish an end goal, including progress monitoring and adjustment

 

REPORTS TO
Deming High School Principal
JOB GOAL
The Transition Specialist serves as a direct support to students with disabilities who are at risk for dropout at Deming High School. The Transition Specialist will use data to document identification, monitoring, supporting and interfacing with students, families and staff. He/she will also address at a minimum four required goals:
1. Decrease percent of SWD truant,
2. Increase average attendance of SWD,
3. Decrease percent of SWD who dropout, and
4. Increase Cohort graduation rate for SWD.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Identification of students with disabilities who at risk for drop out
Build rapport and empathy with students who are at risk
Implement and manage the Early Warning System
Meet with students and families on an established timetable
Collaborate productively with all staff to make sure interventions are in place and appropriate for students at risk

Participate as a member of the Transition Team
Collaborate with DVR Transition Specialists to support students
Make home visits as necessary
Must be self-directed and motivated and hold self to a high level of accountability
Must maintain current and relevant data on all students with disabilities who are at risk, to include tracking and monitoring progress (Increased graduation rate/ decrease dropout, decreased truancy/increase attendance, decreased discipline events, decreased failures)
