QUALIFICATION Associate degree or higher. 48 hours college course, perferred Pass the Paraprofessional test. A complete application on file. 3-5 years experience in Community Resource or Social Work Employment Fluent in English and Spanish
REPORTS TO Deming High School Principal
JOB GOAL Provide assistance on a part-time basis as an on-site Teen Support Specialist (case manager) to the DHS GRADS Teacher in implementation of the activities funded through the NM GRADS Plus: Making Connections for Success Grant
ESSENTIAL DUTIES AND RESPONSIBILITIES ? Recruit pregnant and parenting teens to the DHS GRADS Program. ? Assist the DHS GRADS Teacher in the administration of the GRADS Enrollment and Exit Forms and other required GRADS Forms including student service plan and resource agency linkage for each DHS GRADS student. ? Assist the DHS GRADS Teacher in the administration of the needs assessment, service planning and coordination of services for DHS GRADS Teens and their families. ? Assist the DHS GRADS teacher to maintain the case management files for all pregnant and parenting teens in the DHS GRADS Program. ? Assist the DHS GRADS Teacher to make appropriate referrals of DHS GRADS students to health and social service agencies and to advocate on their behalf for such services. ? Collaborate with the DHS Teachers on behalf of the DHS GRADS students during pregnancy related absences and maternity/paternity leave, to ensure make-up assignments are available to support GRADS students to stay current in their classes. ? Locate and establish relationships with community agencies that work with young parents and their children by building partnerships to help support the needs of the teen parents at DHS. ? Attend required GRADS and GRADS Plus trainings/meetings and other trainings as appropriate. ? Maintain current Inventory of GRADS and GRADS Plus resources and supplies provided by NM GRADS. ? Perform other related duties as required.

LANGUAGE SKILLS o Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. o Ability to write routing reports and correspondence. o Ability to speak effectively before groups of people.
MATHEMATICAL SKILLS o Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. o Ability to compute rate, ration, and percent and to draw and interpret bar graphs.
REASONING ABILITY o Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. o Ability to deal with problems involving several concrete variables in standardized situations.
